My theory is that Ian still doesn`t get a proper treatment for his illness and is in a manic state (would make sense because of the stroller still being around) and this will be a wake-up call for him or sth
yes i was thinking that too because ian wouldn't just be reckless like that and have sex in an alley with two babies around if he was getting help and it also plays with an earlier scene that said that ian's in denial and him doing this and then going to hang out with mickey after could be his moment of realization that he's not okay

I didn`t know him before so I was surprised:) He kind of looses his boyish looks in season two but hes still one of the few guys I like find genuinely attractive. also hes gonna make you very sad at some point so yeah:)
