There’s a distance between them, given the way they speak, especially Dio. Though Dio has no idea who Giorno is, never meeting him, he feels some sort of connection with him despite it. It is known that the Joestar bloodline has a deep connection that spans over generations, and it’s very likely that the ‘closeness’ he feels is due to that, but it’s juxtaposed with him also being Jonathan’s son. 

Dio hacked his way into the bloodline when he stole Jonathan’s body, and while he feels connected to the others, being able to sense them, he is not one of them, it’s artificial. Giorno is both Brando and Joestar, a biological mix of two different families that have their fates entwined with each other. It is probably a strange sensation for Dio, seeing him, being near him with all this in play. 

It also speaks for Giorno in general; close but far. His family, who should have been the closest ones to him were abusive or absent. The gangster he helped as a child was far from him, but he felt a closeness enough to inspire him to follow in his footsteps. People, schoolmates and the community acted close to Giorno, but mostly it was out of fear of that gangster, so he could never truly be close with them either. Giorno’s always been something of an outlier, right from birth.
