ERLING R. - LIVING TO TELL THE TALE
Erling R. was working in Cary, MS separating cows with his son when he began feeling strange. “I was getting very, very tired,” Erl says, “And I started feeling a little pain in my chest.”
Erling had quit smoking the month before and thought the burning in his chest was his lungs aching from the withdrawal.
He left early to drive home to New Orleans to rest. Shortly after falling asleep, he was awoken by an immense pain in his chest. “I decided I was going to drive myself to the best heart hospital that I knew of,” Erl recalls.
Upon arriving at Ochsner, Erl was in cardiogenic shock, hypertensive and in congestive heart failure. At that time, he was treated with an Impella, a state-of-the-art heart pump, and his blood pressure immediately stabilized.

“My father, his father, had a massive heart attack[s] and [they] never lived to talk about it. I was the first one of us to tell our story afterwards,” says Erling. When the Impella was removed, Erl asked if he could keep part of the device as a souvenir. “I have it at my house, on the wall, in a frame,” he says, “The technology saved my life. I really treasure the time I have and look forward to a great future.”
I just saw a commercial for the impella 2.5 and suggesting people to ask their doctor about impella assisted PCI. Idk why ABIOMED would feel like they need a commercial... Doctors shouldn’t have to be told to use an impella the interventional cardiologist should already know about impellas....

A multi-device approach, encompassing active #leftventricular (LV) support with Impella, is safe and can significantly improve survival in patients with cardiogenic shock.
Mechanical circulatory support (MCS) plays a crucial role in the treatment of #cardiogenic shock.A combined multidevice approach may overcome the limitations of each form of support.
The present study investigated the role of percutaneous Impella device in the contemporary MCS armamentarium.The study was done in 22 patients with cardiogenic shock of heterogenous etiology.
They were treated with the following combination of MCS armamentarium. 4 patients were treated with #Impella alone8 patients were treated with Impella and IABP6 patients with Impella and VA ECMO4 patients with Impella, IABP and VA ECMOThe study suggests that the strategy to increase the survival of cardiogenic shock cannot be relied upon a “one size fits all”.
Systematic application of LV venting with Impella pump strikingly increases the survival rate of #cardiogenic shock patients.
Overall complications were few despite of the invasive nature of the CS management.
Overall survival was 73%. However there was a statistically significant improvement in ejection fraction and mixed SVO2 during Impella support...
