While I can see Jax as being a trans woman, I don't see him realizing it in the circus unless they were in there for a very very very long time and he went through some major revelations about the way he treats people.
Honestly I think it'd be more in character for him to abstract and in The Void think "I wish I was a girl." Or for him to realize a long time after they leave the circus.
