I really have to stress that one of my favourite characters in my campaign has the most stupidest death: He drowned in a poisonous swamp because he was pushed by his wife who could not stand him. He then proceeded to pull her in as well and got damned as a wraith for eternity.

Lian’s “getting betrayed by a friend and shot in the chest then half eaten by a demonic entity” death may count as well but he got better.
