The movie already shows that it did not perfectly fulfill their wishes.

Naruto gets the family he never had, but his parents call him “Menma”. This is the reminder that he does not belong in this world.

For Sakura, not only her parents are seen as heroes by the village, but when Chara/Sasuke gives her a rose, I would have had him also ask her out on a date as the cherry on top.

The euphoria would wear off when she starts having a taste of the too lonely and too silent lifestyle of an orphan like in the movie.

Of course, she would use her date with Chara/Sasuke—the date she has always wanted since she was a child—to ignore those feelings, but her father’s face on the Hokage Rock looming over her, as if judging her and not letting her forget that she is not part of this world and more importantly, that her current loneliness is the consequence of her own wish.

Meaning this…

That would have been a parallel to Minato’s stone head reminding Naruto of what he never had.

Chara/Sasuke would notice her discomfort and her attempts to not look at the Hokage Rock and show sympathy for her pain, acting as the caring boyfriend she had always wished him to be. However, he would then say that it was a good thing that her parents died as heroes instead of living to nag her for every little thing and tell lame dad jokes.

It would be a call-back to what Sakura said to Naruto at the beginning of the movie.

Now here’s a Sasuke who validates those words…and she would realize how foolish and thoughtless she had been to verbalize such things (maybe after a flashback of when Sasuke chewed her off the first time for expressing how nice having no parents would be). That would give her the wake-up call needed to forget about her date with Sasuke and to go look for a way back to the real world with Naruto, because it would finally hit her that staying in this alternate reality means letting real people, especially her parents, die.

Her lesson is still “Be careful what you wish for.”, and Chara/Sasuke is what she has to sacrifice (along with the prestige of being the child of heroes). The loss isn’t as emotionally poignant as Naruto’s, but at least we would see a limit to her selfishness, that she does have some standards.
