WHY YOU MUST FOLLOW-UP - This can be boiled down to 2 reasons. First one is technological/process - sometimes Technology cannot be trusted - the Job board, email or fax did not go through as expected. So it is in your best interest to follow-up on the job you have applied for. Secondly, the act of following-up showcases your diligence & organizational skills. So it is crucial to follow-up especially if it is a company that you are interested to work for.
KEEP IT SIMPLE - I like the examples provided by the author in the link. Speak to the person or the company, and ask for status. Sometimes, they have put the position on hold, or they hired someone internally. In this way, you can get closure and move on the next best opportunity. The purpose of this contact (call/email) is not for you to get the job - but to check your status - to see if you are still in the race.
