Macrostomia โ Bilateral Reconstruction of Angle of Mouth This little girl was born with an abnormally wide mouth, which was diagnosed as macrostomia. Her parents were advised to get this addressed at a specialty facial deformity correction hospital. Their extensive inquiries led them to our hospital for corrective surgery. This deformity arises from facial clefts that can form on either side of the face. It very rarely occurs as a part of hemifacial microsomia. Surgical correction of this deformity not only normalizes facial esthetics, but also helps in development of normal speech. Dr SM Balaji, facial reconstruction surgeon, examined the patient. She had bilateral macrostomia with involvement of bilateral modiolus. He explained the treatment plan to the parents in detail and they consented to surgery. Markings were first made to ensure that surgery would establish symmetry of both sides of the mouth. Orbicularis oris muscle was identified and reconstructed. This was followed by identification and reconstruction of the buccinator. A vermillion return flap was then raised to reconstruct the vermillion and ensure mucosal blending with the skin at the commissures of the mouth. Three layer closure was finally performed to ensure that there was good balance between the contour of the mouth and cheek skin. Her parents were extremely happy with the results of the surgery. This recreated perfect lip morphology with good functional results.
