These two were practically betrothed from the moment they took their first breaths. Cersei was quick to reach out to her old friend and ask for a marriage arrangement to be made between their two children.
Their marriage is still a relatively new one but their relationship had blossomed long before then as childish affection between two children. Elsbeth doesn't condone everything Joffrey does, but she does care about him to an extent and he cares about her as much as he can care for anyone other than himself.
