We have precisely zero indications that canon Itachi is cisgender, whether through physical evidence or narrative evidence. No shirtless shots, no onsen shots, nothing. He remains fully clothed whether living or dead. Even his childhood starts with shots late enough that he could have already started presenting as male, given his self-awareness.
We have almost no indicators of gender physically, whether through primary sexual characteristics (genitalia) or secondary ones (breasts, etc.).
EXCEPT TWO THINGS
Itachi doesn’t have facial hair (which, okay, he could just not grow it very well, or he shaves really closely). And...
Itachi doesn’t have an Adam’s apple, guys. You could blame it on the art style, sure, but... no visible Adam’s apple. And that’s the only discernible secondary sexual characteristic we have any view of.
...There’s more evidence in favor Itachi being trans than not, tbh.
