ABVD alone versus radiation-based therapy in limited-stage Hodgkin's lymphoma.
Chemotherapy plus irradiation is an effective strategy in the treatment of stage IA or IIA nonbulky Hodgkin's lymphoma but is associated with late treatment-related deaths. Chemotherapy alone may improve survival due to fewer late deaths. In this randomized trial 405 patients with previously untreated stage IA or IIA nonbulky Hodgkin's lymphoma were either treated with ABVD (doxorubicin, bleomycin, vinblastine, and dacarbazine) alone or with subtotal nodal radiation therapy (midplane dose of 35Gy), with or without ABVD. The target volume included all supradiaphragmatic lymph-node regions (mantlefield), the spleen, and paraaortic lymph nodes. At 12 years, ABVD therapy alone, as compared with treatment that included subtotal nodal radiation therapy, was associated with a higher rate of overall survival owing to a lower rate of death from other causes.
However, the results may not transfered 1:1 to the current situation inHodkins lymphoma since subtotal nodal radiotherapy has been removed from all modern protocols.
