Damon was 23 when he officially lost his virginity to Katherine Pierce. He had a history of fooling around with local girls, but was frightened enough of his father's wrath not to sire a bastard and shame the family.

It was incredibly significant though, with Katherine he had to work for it, beg for it, and in the end he decided a child would not have been unwelcomed. This was before she revealed her nature of course, but by then he was so deeply in love that this was just another thing about her that he could adore.

Damon is bisexual with a heavy preference for women, he flirts with everyone but doesn't generally seek out sexual encounters or relationships. He fell in love with Enzo while imprisoned a St. Augustine's and it was the first and only time he fell for someone without any meaningful physical contact. The entire experience left so many scars that it's easier to forget the whole thing than it is to pick out the emotional breakthroughs he'd had on the side.

Pretty much all of that work was undone the moment he escaped - leaving Enzo for dead.
