I kinda HC that Leo and Donnie were not far off turning 17 in the movie.
I HC the twins Birthday is in August, Raph's is in May, and Mikey's is in June. Which I guess means the movie happened in May/June-ish
Seeing as LHBHs is between 9-10 weeks later - in this AU before the accident the twins were newly 17, Mikey is 15, and Raph has just turned 18? Maybe?
Lil Donnie is mentally 17, but compressed into body not so good at regulating emotions (RIP his bad boy image). His memory is a little glitchy too at times.
Physically he's turned into like 5/6ish?

In the little hands big hearts au, is it harder for Donnie to pull all-nighters in his lab now that he's so small? I imagine he's going to want to fix this whole situation asap and get to work on it right away, but his small body tires so much faster. Does this cause his Ninpo to drain faster?

His attitude to self care has not improved at all ha
He doesn't really remember exactly what he'd been working on when this all started, but he'd been exhausted before the accident and it carried through.
