This part can be read multiple ways. Natsume is afraid that once he leaves the hole, the positives of his life will end. Madara asking natsume if hes going to hide down there forever can directly be understood as “are you going to live in your past forever?” Although Natsume wants to cherish the positives in his current life, he often regresses back to his past.

Even though natsume cannot get up, he still wants to. He just needs help, whether it is from humans or yokai...
