I always need to rationalise how the two would realistically meet so to answer your last point as well I think maybe the Kamimuras would move before The Incident, maybe even because Mamamura knows that guy is creepy af and wants to move on with her life, and so they live near Ken coincidentally and all live yay.
Ken would be a year above Kazutoshi, so they're most likely to meet at a park or playground. Considering Ken was pretty much the same as a kid, Kazutoshi would've probably saw him to the side reading a book and ask him about it, to which Ken would passionately talk about whatever it is. We know Kazutoshi loved Ken's trivia, so why wouldn't he as a kid?
From personal experience (and just a common autistic experience), Ken would probably latch onto Kazutoshi hard. Like always hanging out with him and standing next to or behind him, and Kazutoshi doesn't mind it because Ken's his friend and he's cool and super smart.
I think Ken would end up being the one struggling more in the relationship. I don't have an exact timeline but I don't think Kazutoshi changes much about Reina dying and Ken's parents getting a divorce, so Ken would probably be upset and struggling in front of Kazutoshi more than in original canon at least (especially if he's not masking yet either). Regardless, Kazutoshi would be there for him (or not, if Ken preferred to be alone). Maybe Ken would be less anxious because he feels relaxed around Kazutoshi.
I think they'd mostly be the same as how they already interact, just incredibly closer. I already elaborated that I don't think Kazutoshi would change much even with his parents alive in this au, so he's probably still blunt and teases Ken but does so in a very affectionate, familiar way. Ken would absolutely still be enamoured with Kazutoshi, willing to do something he'd be uncomfortable with if Kazutoshi wanted to do it. Okay now speaking of imagine Kazutoshi teaching Ken how to skateboard.
I like to think they'd go to each other's houses a lot (bonus points for their parents getting along and pointing out how close they are). It's also likely that if they live close enough to each other they're probably going to the same school.
Also Kazutoshi's hair would still be black hold on. He dyed it because he hated how much he resembled his mother but that was because she died. He could want to dye it out of teenage rebellion maybe but I like the idea of him asking Ken what he'd think of it and Ken almost passing out imagining Kazutoshi with a different hair colour.
Oh god the yearning. Both of them realising eventually that they're gay — probably both realising after crushing on the other. Both of them terrified to confess because they don't want to ruin their long-lasting friendship over it...

This ask weirdly relates to another ask I've been meaning to answer.
I feel like saying he'd be the opposite of his traumatised self is a bit dramatic. Like obviously Kazutoshi isn't going to be as severely depressed or so insistent on his independence/lack of aids but there's more to him than his trauma. It's kinda hard to tell what to think because obviously we never actually saw his parents or what they're like, but we'll assume for this that they would've been super loving since that's how Kazutoshi saw them likely.
I guess he wouldn't be as blunt or rude to people, but I still feel like a lot of Kazutoshi's charm is his asshole-ness. Maybe he wouldn't be as easily annoyed, but I still think he'd be the type to banter and tease easily, just not as serious.
I still think it's important to remember that Kazutoshi is still disabled and gay, dead parents or not. I forgot the timeline, but I think the loredump suggested his main disability as a kid was crohn's and then he developed more on account of the suicide attempts. Ignoring the other ones, he still has crohn's, and obviously he's gonna struggle with his disability. Maybe since he has his parents to help him his whole childhood, he might not be super put off by support like he was shown to in canon and maybe not have internalised ableism, but I still think he'd want his own independence (coming from experience — I'm very close with someone disabled with a huge support system but they still hate asking for help most of the time). But, he might still be somewhat pessimistic since his disability can hurt him and hospitalise him, maybe also causing him to not go to things he's excited for because of a flare up. Disabilities can still impact his life a fair bit. Plus, he might be lonely or bullied because of it.
Then there's him being gay. We have no idea if he moved schools/locations or not after his aunt took him in, so there's still much of a chance Isao is still in the picture and outed him. Being a gay teenager and having the one person you trust (non family) out you to a school that already bullied you is still absolutely terrifying. Kazutoshi's parents still being alive likely won't change Kazutoshi getting bullied either. It would still traumatise him, and might make him as closed off as he is in canon, with his family as an exception (or maybe not).
I can't really imagine his parents being alive suddenly giving him way more friends or not making his disability still affect his mentality. I wouldn't be surprised if he was maybe still depressed. Maybe he is happier, maybe he is more trusting, maybe he is way nicer, but I kinda see Kazutoshi as mostly the same — likely just not as severely depressed or with as many disabilities.
Uh to put it simply: I don't think he'd change that severely considering his other traits affecting his character, but it's hard to tell because there's a lot of his past we lack details on like what his parents were even like and how he'd interact with them as a teenager, plus I hate almost everything von has to say about him because her writing for him was so fucking ableist.
I don't normally ever get into top/bottom discourse because I believe in nuance and getting to explore the characters through different interpretations. However ☝️ I'm a firm Ken/Kazutoshi believer on account of Kazutoshi's cataplexy and that's literally it.
