For my Ursa yes she did change her face to flee Ozai and remain undetected however a mercy in the deal is those that truly love her soul can see her as fire lady Ursa.

If Ozai were to find her he’d see a lowly peasant and not the former fire lily of the nation. A major reason she fell truly in love with her second husband and had kiyi is once he fell in love with her he saw her as the missing fire lady Ursa and instead of giving her away to Ozai who would of rewarded him greatly he chose to keep her secret chose to love her.
