I find it so important that Tsukasa is allowed to feel resentment and anger for being forgotten and left behind to the point it almost infects him
Not resentment towards his parents
Not necessarily jealousy towards Saki
But rather emotions that every child feels because it's normal to feel that way and no child can ever be as mature as an adult. He and Saki were just children forced to mature too quickly because of the circumstances and it's heartbreaking to see how innocence is stolen away from them because of sickness.
Tsukasa loves his sister so much and he always wants the best for her, and even as an elementary school kid he had to be mature, strong and sensible for Saki's sake, because he knows his parents are not doing it on purpose and he gets heartbroken at the thought of her condition worsening.
But the thing is, there's only so much a kid can take before breaking down. No matter how strong and logical he wants to be, in the end Tsukasa gives into his petty thoughts and I think that's beautiful because that's a part of being human and no human being has ever managed to put away their needs for other people's sake without reaching a breaking point. He makes petty and childish thoughts about his parents even if it's brief and it's cathartic, because again, he wasn't even ten years old. If he didn't have any petty thoughts back then, then when was he supposed to?
Saki has spent half of her life going back and forth in hospitals, constantly worrying she'll relapse like no child deserves. Tsukasa understands that and is more than willing to forget about his feelings for her sake, because he wants to be a good brother and because he genuinely cares
But that's the price children who grow up too fast have to pay. Being deprived of a normal childhood leaves deep scars on people and even they try to repress the memories, one day the wounds will open again and you'll have no choice but acknowledge the pain
