Maybe the pilot knows the port to be controlled by a ruthless organization whose agents will take advantage of any perceived weakness. When they set off to seek medical assistance it soon becomes clear moving without attracting attention is going to be impossible. On top of the infrequent contractions the stowaway is now having he’s also suffering through back labor due to the first baby resting deep in his pelvis facing the wrong way. Instead of taking detours through quieter streets they agree he’ll hide in a disused storage building while the pilot finds a ride (but perhaps gets in some trouble in the process, leaving the stowaway to labor alone longer). The stowaway struggles to keep quiet and has to swallow his moans of pain on several occasions when the buildings surroundings turn out to be busier than they thought. And who knows how long he’ll have to endure it on his own...

Or perhaps something else I haven't yet cooked up...

I might come back to this and refine it later, maybe...?

I don't think I ever posted this and I guess the tiermaker got taken down again because I could not find it... But here's mine from some time ago ✌️

When you didn't pack enough clothes for a months long trip. And you're swelling twice as fast as expected.

A guy stows away on a small cargo ship to escape a bad situation on a remote colony planet after finding out he’s pregnant, intending to spend the couple weeks of warp travel hidden among the containers. A few days in he has some concerns: the ship’s engine sounds off and the pilot, the only crew on board, seems to be a crude but sharp smuggler keen on security. When the man is eventually found he learns that due to the poorly maintained and defective engine he’s stuck in warp travel for months instead of weeks with a cranky stranger who’s very unhappy about his less than legal business being uncovered. The atmosphere is tense and cold for a while, the man worrying if he’ll have to deliver the baby on his own before the ship drops out of warp and the pilot contemplating how to keep the man quiet about the smuggling operation once they reach civilization. When the pilot realizes the man is pregnant his attitude slowly begins to soften and their relationship shifts from antagonistic to tolerant to affectionate as the man welcomes the fumbling but earnest attempts at support and tenderness. As months pass it’s starting to seem more and more likely the man may go into labor before they reach their destination...
