So his “ugly” is more about his new ugly diabolical nature which Light Milk is trying to accept (you know like Lucifer used to be the most beautiful of angels) and develop into something new, something acceptable - and after he is healed and accepted by old friends he no more feels that ugly (still dismorphophobic though) and his acceptance of his new self will result in changing how the others see him. So he will be no more ugly for them, although he didn’t change. Also it’s a little bit about how a messy neglected person with tired face and misnurtured body may seem terribly ugly but will bloom into a handsome one after it gets some proper care.
