it took another trans man sharing his blood test results with me for me to realize my doctor was committing malpractice for the past seven years by giving me far too little testosterone
it took many trans masculine people sharing their results with me to confirm it so I knew to try to find a different doctor, and they are now doubling my testosterone
I'm going to make sure to warn as many trans people as I can, as an individual, so that other people were not hurt in the way that I was hurt
this is why it's important to build communities for trans people and include them
To add to this point: It took me three years to realize my doctor committed malpractice by massively overdosing me on testosterone. It took me collapsing on my way to work and not being able to use my legs for them to figure out I was about to have a stroke as a direct result. My blood being as thick as it was meant even with twice daily blood thinners the phlebotomy I needed wasn’t possible with a 16G needle. I had a McDonalds strawberry milkshake running through my veins as apposed to the blood that should have been there and I just wouldn’t bleed. No matter how hard they tried they weren’t able to draw enough blood.
In part due to the autoimmune response that alerted me something was wrong I’ve been in pain everyday since the day before I collapsed. It’s been 5 years.
I had to be taken off testosterone and placed on blockers multiple times to prove that testosterone at healthy levels wasn’t contributing to my symptoms continuing. As an additional “fuck you” we already knew my body doesn’t function well on an estrogen dominant system. I required hospitalization and medication when I had periods when I was off testosterone. As a result of that it was recommended I get a hysterectomy sooner rather than later. Now I’m back on testosterone off blockers with a few less reproductive organs and a shit ton of medical trauma.
I will continue to scream this story from the top of my lungs until people realize the importance not just of testing your levels but understanding what the hell they even mean.
Talk to your friends about your levels have them talk to you about yours. Remind each other when it’s time to get tested. Ensure you and your friends don’t wind up like me.
-between ~300 and 600 ng/dl is the average male testosterone level across men across most age groups (excluding under 18)
-average male red blood cell count is between ~4.5-6 million red blood cells/mcl
-average male hemoglobin is ~13.5-17.5 g/dl
testosterone can cause your body to produce a lot more red blood cells (polycythemia) which can cause the blood thickening ^ not only should your doctor be checking your testosterone levels AT LEAST once a year, but they should also be checking your rbc count and hemoglobin levels. your doctor should also be sharing these results with you so you can make educated decisions about your health. please ask your doctor to see your results if they dont already share them, and if your levels are off, please try to see a new doctor if possible 💚
also, not super related but every time gender-related medical trauma comes up i gotta share my shit: if you want to stop having periods/have lighter periods, but don’t want to go on testoterone, there’s birth control options that can help w that, such as the Mirena and Kyleena IUDs (there used to be one called the Skyla, but they don’t make it anymore).
I am nonbinary and while I’m not transmasc and don’t take testoterone, periods were causing some serious gender dysphoria for me (among other health problems). I had severe anemia from how heavy and long they were so I needed the IUD to survive, but the dysphoria going away was a much needed side effect. I didn’t realize how much of a mental toll it was taking on me to keep going to the stupid fuckin “feminine products” aisle at CVS until I didn’t have to do it anymore. I straight up don’t get periods now.
it’s not for everyone, and you should talk to a trusted physician about it (and/or check it out on scarleteen or planned parenthood), but I wish I had heard about it sooner (both so i didn’t have to go to the emergency room and bc i hated that shit).
sorry to derail the post - I try to share what I know whenever possible bc sex ed varies so wildly, and information about gender affirming processes and birth control can be inconsistent.
if you decide to get an IUD, I’ll share some of the things I learned (and things that should have been told to me that weren’t) below the cut. It’s a lot. I live in the US, in a left-of-center-but-not-too-left area in the Northeast, for context.
You should know that the process for getting an IUD inserted can be painful. They will tell you that it shouldn’t be painful, but what they actually mean is that for the majority of people it isn’t painful. A good litmus test is if pelvic exams/pap smears are painful for you. if they’re not, congratulations! this all probably doesn’t apply to you.
I mention this bc I imagine that for many trans, nonbinary, and genderqueer people, vaginal penetration may be dysphoria inducing (it is for me at least), so maybe you’re not doing it on the reg, whether for sex or for tampons or kegels or any other reason.
This is significant bc when they ask if you are sexually active, what they mean in this instance is “a. could you be pregnant but more importantly b. are you regularly engaging in vaginal penetration during sex” bc given that the vagina is elastic, things regularly going into it tend to loosen it up a bit, so the insertion of an IUD would be easier*. (yes, it is bullshit that they phrase it like that).
They’re not usually asking about:
clitoral stimulation
outercourse
pegging
cunnilingus
roleplay/kink that does not involve vaginal penetration
so even if you consider yourself to be sexually active bc of your engagement in those acts, they may not.
If you use a regularly use a dildo/penetrative sex toy to masturbate, but don’t consider yourself sexually active bc of the lack of a partner, they may still consider you sexually active, so it may be good thing to mention.
Now, this all might be totally irrelevant to you. Vaginas come in all shapes and sizes, and if a pap smear/pelvic exam isn’t painful for you, this probably won’t be either. There’s plenty of trans/nonbinary/genderqueer people who are fine w vaginal penetration, and plenty of people with vagina sizes and elasticities that would make this process totally painless. I don’t say any of this to scare anyone off of getting the IUD - it literally saved my life.
The reason I include this is so that anyone who ends up in my situation, where pelvic exams/pap smears are extremely painful, they can know all the things I know. In eir book Gender Queer, Maia Kobabe discusses eir experience with pap smears, and reports similar pain (along with similar dysphoria cause by vaginal penetration) so I have reason to believe it is at least not just me. (also warning: the stupid wikipedia page misgenders Maia Kobabe. apparently it’s wikipedia policy to use they/them for people with neopronouns? fucked up)
They CAN knock you out for the insertion of an IUD, or for a pelvic exam/pap smear. It’s “unusual”, but they can do it, though you’ll probably have to go to a hospital for it, and it’s more of a hassle bc going into surgery has a bunch of rules to follow (for you and for them). You will need at least one other person to know you are getting it done, bc they have to drive you home afterwards. My insurance covered it luckily, but since it counts as surgery you may want to check on whether it’s covered by yours, or what the policy is at the hospital in question. If pap smears/pelvic exams are extremely painful for you, I would highly recommend it. If you do decide to do this, you might have to push for it a bit, but it was extremely worth it for me. not believing “““women”““ is ingrained in the medical industrial complex (gross) - advocate for yourself and your body!
Short of knocking you out, here’s some other things that were recommended to me. I can’t say they helped, given that I pretty much blocked out most of the first procedure* and remember very little of it, but they are suggestions given to me by a gynecologist. obligatory “i’m not a doctor and am not qualified to give medical advice”
warning them that you have a smaller-than-average vagina (the idea of an ‘average’ vagina is completely made up, but they believe in it, and sometimes you have to work w the system in order to game it)
asking them to use a pediatric speculum (it’s smaller) - I do remember this making a significant difference to me.
asking them to warm up the speculum beforehand
asking them to start with fingers and work up to the speculum
taking a LOT of tylenol (acetominophen) beforehand. as in 1000 mg every 8 hours for 24 hours beforehand, and then combine it with advil (ibuprofen) 30 min before the exam.
they can prescribe you a “cervix softener” to take the morning of
dilation exercises - talk to a trusted physician about this. here’s what scarleteen has to say
doing your best to relax - counterintuitive, I know, but if you got a happy place go to it, relaxed muscles make it a lot easier.

*this info mostly comes from discussing what was meant by sexually active with the gynecologist, but I will admit that part of my source for this information is anecdotal. I think it’s important to include the anecdote in question as an example of potential attitudes you may face in the medical field, but it’s a bit fucked up so be warned - here it is:

The assistant gave a sympathetic “ohhhh” like that was unfortunate but explained everything.
