Here's the thing, though. I wrote my post with intentional word choice. I never once said that Anna said "I am a lesbian". I said that she self-identifies as one. Which she does in the following panels after she said "Does that mean I'll be a lesbian?"
The expression she makes here is the confirmation I was talking about. That's when she confirms that she's a lesbian.
It's like if a group of friends are deciding on what to get for dinner and pizza starts getting thrown out as a suggestion. One person might say "are we feeling pizza, then?" because it opens up the conversation to actually confirming that's what they want.
It's very similar in this context. Suuichi tells Anna that she wants to be a girl, Anna asks if that makes her a lesbian, and they confirm she is with nonverbal communication. Sometimes people will ask questions to confirm something they already suspected to be true and that's what was happening here.
Not relevant to the general topic of conversation, but "I want to be a girl" is, by the way, an exceptionally bad translation; in Japanese, Shuichi clearly said "I want to be a woman," (女の人, not 女の子).
And this is precisely what is emphasized in the scene - Shuichi emphasizes to Anna that her desire for a gender transition has not disappeared after she became an adult. The thing is, throughout the manga, Shuichi kept saying "I want to be a girl," but in the finale chapter she said "I want to be a woman."
Shuichi makes it clear that her female gender identity isn't a phase, it's something that doesn't disappear with age.
And this phrase definitely provokes Anna to reflect on how she will label herself after this.

One of the saddest tendencies of the men's rights movement is its sympathy for so-called "equity feminism."
