My heart rate is 36bpm and I feel very sick. What should I do?

Honestly I keep forgetting I have this aha. I've just been having so much work and then all my medical problems and stuff. I'm really sick and underweight. I have congestive heart failure, and it's so scary. I barely eat but I'm not hungry either.

Sorry i haven't been on here much, I have been busy with work and boyfriend and stuff.. I ended up breaking up with my boyfriend last week. He called me fat(unintentionally)and just wants sex and drugs, i feel used and pathetic, worthless, horrible.. Ive been diagnosed with adhd as well so im gonna start meds this week. Ive been a junkie these past months getting my hands on whatever i can get. Weed, cocaine, amphetamines, and meth... I stopped everything and am gonna start taking adderall as i used other drugs as a way of coping with adhd. I probably depeloped a eating disorder from taking a new medicine called topamax for my headaches i hate food and it tastes bad. I'm 86 pounds and 5'8 my parents are scared. Life seems to be going downhill again....
