imagine if you will, someone who might kiss themselves as a way to self soothe, maybe to ground themselves with a specific sensation. it's always in the same spot(s), and so that area becomes covered in light moles (I'm thinking the back of the hand personally). when they get a partner however, they have a second person to soothe them, and during a bad panic attack or something their partner kisses that spot before the first person can/instead of the first person, and the mole that appears is drastically different. it pops out in comparison with the others bc it's so much darker, clearer, than the wall of moles that were originally there, and the reality sets in that theres someone to help them so much more now, who cares about them

~someone who's cut themselves might kiss those areas in the future as a way to be kind to their past self. a possible theraputic method could be to kiss the area they want to cut as a way to curb the impulse, to keep track of how many times they successfully avoided breaking a streak (applies to addictions too). suicide survivors, or those contemplating suicide in this world may do something similar, remind themselves that they're loved and important, making that reminder permanent. getting a tattoo surrounded by birthmarks left by loved ones who were so, so scared they lost that person for good, because it reminds the survivor that they have something to live for

maybe someone felt young love, or had an incredible partner, but lost them too soon. all they have left is the moles/birthmarks they left. in memoriam, they get tattoos highlighting the marks, making sure they, and the one they loved, are never forgotten. they don't allow others to kiss them in those spots, the tattoo a reminder to keep that area sacred in a sense, to only contain the one they lost

and maybe it doesn't just have to be marks from romantic love. imagine the above scenario but with a close friend, or a parent lost too soon. imagine seeing that nonromantic person on their deathbed, and to make sure you never forget them, you ask for a kiss on the hand or arm. the tattoo design circling that one, necessary mark.

there could be people who reserve a spot on their bodies for every person they may lose. preemptively, an area forged by grief, the birthmarks intermingling with their only common factor being that their creators have been taken by time.

*some asshole could pull up and kiss that area, marring the memory of your dearly departed.

consider that if people kiss family in this universe, it's then uncommon to see someone without moles, because it doesn't just mean they haven't been in romantic situations, but their parents never kissed them or doted on them. certain areas are seen as platonic/familial locations for moles, while others imply romance

although i imstead prefer that there's some sort of cultural rule that kisses on the skin are reserved for those you (romantically) love most, because of their permanent nature. non-romantic kisses are only reserved for clothes and in rare/premature cases, death. When there isn't clothing in a specific area, you use some sort of barrier. for example using a cloth or paper towel to kiss a hurt kids knee to comfort without giving them a mole. kissing the top of the head is something that particularly affectionate families opt to do in important moments, bc hair will cover those marks

secret relationships are hidden through makeup, or kissing your partner on the scalp where no one can see (much more common with those not allowed to use makeup imo)

*what if someone finds themselves at a bar or party, their drink is spiked and they wake up to moles that they know weren't there before. it's painfully clear what happened and it makes it hurt that much more. people who've had birthmarks from a young age not out of love, but out of abuse use makeup to cover it up, or maybe show it as a way to spread awareness

*people who regret certain relationships, certain professions, certain choices, also use makeup to cover up something they're ashamed of.

*someone with a lot of moles and hates their presence, tries to get a tattoo to permanently cover it up because the makeup is exhausting, or they want to make their own beautiful permanent mark (the tattoo) as a way to reject (or possibly try to accept/create good from...) the ones forced upon them. they may be told that the moles are too concentrated, and no tattoo designs will be able to cover it up safely (you shouldn't tattoo over moles if im not mistaken bc it could make it harder to notice skin cancer)

a dear partner has a bunch of moles, so a new one popping up goes unnoticed because it simply blends in. then much too late they realize it's skin cancer and there's not much that can be done to save them now

what about those who choose to embrace the moles they have by putting colorful dots where the moles are, yknow cause whimsy.

*additionally what if putting certain colors/patterns of makeup on/around a mole, or a certain type of tattoo nearby indicates different meanings, family, partner, assault, etc.

there's people who fake moles to fit in, kinda like how people lie abt not being virgins or having partners to not sound lame (i really don't get that but ik it happens)

it could be that kisses on the lip are only for marriage ceremonies in some cultures/religions, so seeing multiple is a sign of shame. or alternatively just the first lip mole is reserved for marriage, for that "you may now kiss the bride moment" and these cultures look down upon those who have their first one, or multiple, before marriage

being born with a mole is crazy rare, and in many religions is seen as a kiss from the heavens, blessed by god, proof that your soul was loved even before you came into the world; different locations hold different meanings and different types of blessings or predictors of a certain trait (imagine palm reading but make it birthmark). unless it's on your lip, or any intimate area for that matter, because now you're impure, forced to walk through life being marred by the beings above, that innocence taken from you before you even had the chance to think. few cultures look down upon any mole from birth, just as few people just don't give a shit if you're born with a mole at birth (outside of the "woah thats pretty rare, neato" reactions that are bound to happen)
