Some ghouls aren’t 200 years old. Fallout 4 kind of ignores it, but fallout 3 establishes that the atomic blasts weren’t the only way to become a ghoul. Pockets of radiation or rad storms, the like. Plus, the prevalence of non-feral ghouls (and also feral) is surprising if they were all 200 years old. You’d think that most would have gotten killed by something, if they were really that old, implying that at least some of them are younger.
