Hysteroscopic Polypectomy : After a hysteroscopic Polypectomy , grasping the polyp and retrieving it is a tedious task , especially if the size of the polyp is large . Enter Tri-pole grasper , this wonderful piece of equipment will grasp the polyp completely and remove it in toto. I usually cut the polyp completely and leave a base so that it doesn't float around when trying to grasp it . I then introduce the tri-pole grasper and retrieve the specimen . TLH : Finding the right plane for bladder dissection.
For a complete bladder dissection , always remember . The fat belongs to the bladder , so when pushing the Bladder down after cutting the UV fold of peritoneum . Push on the fat , to aid this dissection you could use a laparoscopic peanut (very very useful ). It's also essential that you get good uterine traction , poor traction would not give you a proper dissection plane . Another useful trick is to identify the uterine vessels laterally , and stay just above the vessels . This will help identify the rest of the plane to dissect (especially in patients with previous c-sections).
Case Points : TLH with difficult bladder dissection
Bladder dissection is probably the most crucial step in a hysterectomy . A few pointers which can help with a good bladder dissection . Do not cut the corneal structures first . Start with the round ligament at the middle (stretched with either the spiral or uterine manipulator) .this will stretch the bladder down , and when you start dissecting with the harmonic , you would find that the plane is created (thanks to the cavitation all effect of the harmonic ace ). This way you will have minimal bladder injury .
