In a nutshell:

In Carla's Dark Chapter, instead of staying quiet in the infirmary while he sucks her blood, Noctis yells and accuses him of crimes that could get him expelled for the students outside of the door to hear. Unfortunately, Shin interferes, lying to the worried group of students and saying that Noctis was having a "manic episode" and that Carla was just trying to calm her down. They left, and Shin entered the infirmary. Carla orders him to latch onto her as a punishment, as well as to ensure that she doesn't escape easily.
