I wonder if folks have started to build permanent structures on or inside of Katara? I imagine people might build simple tents and small houses on her hips and butt, with maybe more elaborate structures on her belly.

There is a lot of "water" inside Katara's belly - her stomach acid, which she perfectly controls, and in which it is surprisingly comfortable to swim)
As Katara grows bigger and fatter her bending has become more powerful, to the point that she is freezing more and more of the world around her. As the ice and snow of the South Pole expands so does the territory of the cold dwelling mega fauna the Water Tribe relies on for food. Already most of the southern hemisphere is plunged into an ice age where Glacier Queen Katara rules supreme.

Naturally, even during the reign of her father, Azula surrounded herself with predators. The daughters of noble lords, voracious soldiers, and even peasants and vagabonds - all big, strong, and competent.

It certainly was a shock to find out she had to put on that much weight but it was part of her royal duty and anyone who said no to the princess well they were soon seen on her thighs
Or just in her stomach - Azula has long learned how to keep her subordinates in her huge belly without digesting them. The very embodiment of complete domination over a person is to reduce him to a small bulge in her stomach, and forget, without even allowing him to become a part of her body.
And if the body doubles does her job well, then she can learn it too~
Survival dictates you do whatever it takes to keep the princess happy with her performance and with how much of perfectionist is the princess was it was a difficult task.
But the reward is also appropriate - Azula demands a lot, but also gives a lot. Faithful service is the best way to stay outside, not inside the Fire Lord's huge stomach. Not to mention the power.
