Caleb has an add-on “Jaw Smasher”, which is literally “The bullet that ripped through Caleb’s jaw during a frenzy[sic] shootout”.

This is the first time that I know of where an add-on directly impacted the physical appearance of the killer due to backstory relevance. His face is twisted because a bullet went through his cheek and blew the hinge off his jaw, which left that nasty scar. Due to an inability for medicine to repair the damage at the time, the muscles on that side of his face died, causing permanent disfigurement.
