I know its just fiction. But is it really if its picturing very obvious csa, torture and organized abuse?

Might only be me but i've experienced child abuse and what i saw in that anime was a little too realistic.

It’s a world that doesn’t hold any punches, and the story is about moving on and growing despite what has harmed you in the past. There’s a persistent theme of “It happened, it can’t be reversed, but I can move forward.”

Also it is usually not realistic. It’s fantastical, like Ozen beating up Reg with her superpowers or anything Bondrewd does. There are things like Vueko’s story that’s realistic and not at all glorified or condoned.

It’s not “obviously working”. The MiA fandom is against the abuse of real people. Akihito Tsukushi recently spoke out about the abuse going on with Shogakugan and an abusive artist. He shared stuff to support the victims.

Fiction is a playground to explore unsafe ideas in a safe environment.

If you can’t handle the stuff in MIA, that’s understandable. I wish some of the shit was toned down. However, yeah, the plot is fantastic, and the characters getting abused is relevant to the plot and themes. I don’t have time to go over it here but I’ve made some video essays about it.
