A bunch of (disjointed) thoughts about this:
I love it when multiple fonts are used to create an additional layer of meaning for a text (Although I am going insane with the introduction of the Times New Roman (-esque?) font at the end; what does it mean aaaaaaaaaaahh).
I noticed that Dess is the only one ever drawn in full colour; even her sister dearest is only shown in flat monochrome. She truly can't trust anyone.
I love how the shape of the cat's head changes subtly to be closer to the cartoony and simplified head of FRIEND.
On page six, Dess' antlers are bent into the shape of the Knight's antler for one panel.
The way the therapist's neck is slowly elongated into (even for this stylised comic) utterly impossible lengths across multiple panels is so unsettling.
The panels in the last few pages are all at an angle instead of straight on the page. Dess, as well as the story, is going off-kilter. (Is this the comic equivalent of a Dutch angle?) Interestingly, this is already the case when the normal colouring returns for the last time as Dess goes outside to smoke. Even in this brief moment of supposed peace, you can already see it go wrong.
Gaster's face being superimposed onto Kris, the character in the main cast who is closest to being "faceless"..
On page seven, if you zoom in on the small silhouette of Dess, you can see that one of her antlers is bent and her waist is rather narrow. Although perhaps not obvious upon first glance, she seems to be in the process of turning into the Knight here.
On the page before FRIEND is fully revealed, you can already see a sliver of pink in the cat's yellow eyes.
The wallpaper in the background of the therapist's office is patterned with X's. Considering the therapist (and the office) is Mancountry-green, the X, as the last step to reach Mancountry, becomes more than just a wall decoration.
Noelle's childish, rounded face contrast so well against Dess' long and angled one.
Dess says outright that she is afraid of the dark. The entire comic is created out of the dark. It permeates every panel. The base/background colour of the comic's pages is pure black, the contents of the comic only painted on top. How paralysingly terrifying of a reality it must be to live in a world which is only an island in the ocean of your fear.

I'm quite happy that what I was intending to write seems to reach the right message. It's very common to see some cases of having a mental illness that usually leads to distrust of most people. It's not just having a mental illness, but also having to deal with your wrong perception of reality and self. Or some delusions that usually are symptoms presented in some mental illness.
It can be seen as a metaphor for mental illness that affects your perception of reality. Unfortunately it's common to have ups and downs of mental health, even though sometimes you see your closed one (Or you) recovering well, then in the next month you see them having the worst mental breakdown.
I'd like to think that the cat can be seen as a metaphor for mental illness, like Asgore, Kris and Carol seem affected by this too, that even their desperation to find her also ironically lead to developing a mental illness, especially if you see it from a different angle.
When her mental illness gets worsening, I had to make sure that most everyone has any distortion or faceless, except Noelle, because she's the only one Dess can trust. If Dess doesn't trust her, she wouldn't open up to her. Even though Noelle doesn't fit in her reality, she's harmless in the Dess' eyes.
Even though I find it a bit saddening when they're often characterized as villains in media content, but in truth, they can be the most vulnerable people living with this fear... This kind of fear can be truly overwhelming. It's often suffocating living with mental illness that alters everything you see, hear and feel-
