I think I’ll let two earlier posters have the first say:

I think this is the result of vacuum pumping, and so is not permanent, at least at its maximum extent, though after a time there may be some permanent stretching. However I do see problems for someone who has a dick permanently 12 inches in circumference, after silicone injections for example. First there are probable social and employment consequences for someone with a large genital bulge that is not possible to conceal. Second, finding someone to fuck could be difficult!

Actually I had begun to wonder if this was genuine, as I would expect his glans to expand more than it has with pumping, then by pure chance I found this:
