Okay, so we know that while Milah wanted to go with Killian, she turned him down because she of Bae, right? And let’s be real; even in the height of his pirate swindling days, Killian could never love someone who willingly abandoned their child like he was abandoned.
In season 5, we see the plot line of how Rumple sold his second born child in order to save Bae’s life. And in that flashback the audience is to take that moment he tells Millah ‘’we just have to not have any more kids!!’’ As the moment she’s done. She decides to leave him. Because, as she says, ‘’you’ve traded away our future!’’
But here’s the thing; if Milah was daring enough to throw her life away, abandoned her husband and son, even suggest killing the merchant in the first place to procure Bae’s cure- why didn’t she kill the man herself? Why ask her coward of a husband?
If we look back to season 2 when we first begin to see flashbacks of Milah, she wasn’t trying to leave him at first. She tries to get him to leave the village; ‘’We can go where no one knows us.’’ This contrasts with that moment in season 5.
So, my theory is Milah didn’t want to leave her family (Bae, mostly Bae). She realized Rumple sold their unborn child to the merchant, tried to get him to gather the courage and leave the village (and outrun the merchant who'd be trying to collect on the debt), and when Rumple wouldn’t budge, finally went to Killian for help.
(It’s my personal opinion that Killian’s taunt of Rumple, asking him to pick up the sword and prove he was willing to fight for Millah, was staged, Milah still looking for further proof he was willing to do what was necessary for their family. But this is based on my own personal observations of character and is less easy to substantiate)
And in the circular narrative that is Killian Jone's life and the love of parallels that fairy tales can evoke, it makes the moment of pregnant Bell staying on his ship to hide from Rumple that much more heartbreaking.
