Transgender jimmy novak.... when cas takes him as a vessel he is aware that his vessel is transgender but never thought to say anything to sam or dean beacause he viewed being transgender as unimportant to his mission.
So the first time cas gets injured and has to take his shirt off around the winchesters, mostly dean is very surprised to see his top surgery scars.
Although neither of them (winchesters) say anything in the moment dean later asks cas about it. (Sam just figures it out beacause hes locked in like that and less dense than dean)
"So... whats with the scars?"
"I don't get scars, dean."
"No- not the ones from yesterday I mean the... the ones on your chest. How did you get them- or i guess how did your meat suit get them."
"Subcutaneous mastectomy"
"a what"
"Subcuta-"
"No- no, I heard you cas- I mean what is that"
"A surgery to remove breast tissue"
"Did uh.. whatever his name is have gyno or something?
"I dont know what that is"
"Its- uh.... nevermind. Why would you even need that?- youre a guy i mean..- you dont even have boobs-"
(Cas tilts his head slightly)
"Dude dont look at me like that. Its your body.. how come you dont know?"
"Dean. I dont see how the "look" of my vessel is an important topic right now."
"Its- its not important no I just, its weird man.. youre an angel i thought like you-.. you can just fix everything so, why?... why not heal the scars?"
"He- my vessel.. doesn't want me to. They're important to him"
"He wants to keep the.... the scars- why-?.."
(Dean stares at castiels clothed chest, squinting with confusion as if the harder he stared maybe he would understand)
"They represent something to him, a kind of.. rebirth. Hes a very devout man- my vessel."
"Right... isint like mutilation or changing your body a sin? I thought God made everyone perfect already or whatever"
"Some believe that, but God doesn't think any less of you for being a mortal and changing your vessel. In the grand scheme of religion or of.. good or bad... in a way change is a necessary part of the human experience. And in the case of my vessel this change was very important to his human experience."
