I went back and watched this episode and noticed all the visual warnings Mami had to warn her not to get so cocky during her battle with Witch Charlotte.
I was really surprised at how many warnings there were visually as well as Homura trying to warn her. Maybe if Madoka hasn't been distracting Mami, she might have lived. :p
