How to smooth out meshes (super quick tip):
In Milkshape: select subset and click Vertex - Cat's Normal Smoother (you'll need this plugin).
Doesn't always look super nice but usually does the job. If it makes too many dark spots, try blender.
In Blender (for blender 2.79b): select subset - press Tab - (look at the left part of the screen) Tools - Remove: remove doubles - Shading/UVs - Edges - Smooth.
Does the best job, but depending on meshes sometimes it's better to select only certain mesh parts to be smoothed out, like only along the seam line (highlighted with a blue line).
