It's about a man who is held on trial. He pleads for an Insanity Defense, claiming that "his brain has claimed its glory over him" and he "has a good heart, albeit insane", which leads to him being sent to the infirmary for electroshock treatment. This is incredibly painful and distressing for him, including lines like "Someone help me understand what's going on inside my mind / Doctor I can't tell if I'm not me" as his mind gets destroyed by it. The tragic irony is that if he was lying about being insane at the trial to get a lighter punishment (and it is commonly agreed that's what happened)… well, after the "treatment" he received, it certainly is not a lie anymore. Adding to the whole Spiral-ness of the song is how creepily surreal it sounds, with the album version of the song starting by playing BACKWARDS before the "normal" version actually starts.
