mAN okay this is just one of those things that kind of happens without any planning, but it makes sense. Int he regular verse, Hugo is aware of Otis’ criminal activities but he finds him one of the least threatening individuals plaguing the Gotham Underground. It’s worth noting that Hugo does not see the rogues as irreconcilably insane monsters, but perfectly curable human beings (with a core focus on Riddler, Two-Face, and Harley) or victims of circumstance (Croc, Scarecrow, Harley AGAIN). Otis just seems troubled to him, and reminds him of a child. 

And, because he reminds Hugo of a child, he feels responsible as both an adult and someone who heals the sick and downtrodden (speaking in one way) before they reach the point of no return (after which they earn final rest). There’s also an advantage of having someones trust - when you earn trust you can ask favors and manipulate without having undue suspicion placed upon themselves. Hugo likes manipulation, he likes being in power - the more friends and allies he has the better. And because he sees Otis as an easily manipulated (rather pathetic) and helpless force he thinks he can shape him into something better.

No doubt he actually cares though, to an extent. It’s going to grow into this....extremely protective bond. Otis needs his help and his guidance to become a better man. So he’ll get it.

There were villains whose names were whispered like curses on the wind, fearful eyes wide and staring about as though the name would summon the criminal. Then there were villains they almost forgot about as those who demanded attention forced themselves into the spotlight of public eye, cried out for fear and attention to be given to them.

Strange made it his goal to know them all – for they all possessed something of interest about them, something which connected them to the wings of the night towering above.
