Nursing Diagnoses Study Guide/Reference Booklet

Nursing Diagnosis Concept Maps

We are given a handful of diagnoses each week to complete into our Nursing Diagnosis Booklet. These are the selected diagnoses. There is one diagnosis per page. I used Ackley's Nursing Diagnosis Handbook: An Evidence-Based Guide to Planning Care 12th edition (ISBN: 978-0-323-55112-0) to make these notes. I will update the Google Doc's link periodically as I type them up and add them to my master booklet.

Each 1-2 page diagnosis has the page number where the diagnosis can be found in Ackley's, as well as listed relevant information such as the definition, assessment, risk factors, defining characteristics, related factors, at-risk population, associated condition, NOC, client outcomes, and NIC.
