The point of the art is that the Wall Monster has replaced all of Johnny's past memories.

Johnny doesn't remember his life before the Wall Monster.

And in fact, the Wall monster controls Johnny, forcing him to sacrifice human blood to him.

Раньше я думала, что буду притворяться загадочной англичанкой и никто не поймет какой на самом деле я национальности и какой у меня родной язык, а потом мне впадлу стало и писать ахению на русском которую поймет мало кто куда веселее

This art is based on my theory, I don't know how much canon it is.

But the theory is that the Moose monster from the wall can talk to Johnny through his voices in his head through Mr. Eff and D-Boy.

Also part of this theory is that I assume Johnny lost his memory due to exposure to the monster from the wall.

In "I feel sick," Sickness is not exactly a the wall monster, but rather Mr. Eff, but their goals are the same to make their owner kill people. Sickness told Devi, "We're taking care of you, protecting you. You can do anything, even kill." I don't think the phrase about killing was just inserted. Devi herself she's ever thought about killing anyone, it's clearly not her who needs it, but Sickness.. And who else needed the owner to kill and smear with blood? That's right, the monster from the wall needed it. If Devi hadn't been able to overcome the Sickness, she probably would have painted the easel with blood like a wall. And if it is not difficult for a wall monster to remove a person's memory, then he may well influence the mind of his victim. And I think he can speak with voices from victim head, too.

Therefore, this art shows how the wall monster has a wall with blood, this is his stage, and his dolls through which he addresses Johnny are Mr. Eff and D-boy. At the same time, Nny himself is also a puppet of the wall monster, there are threads above Johnny's head, for which the Wall monster pulls and rules Nny through them. Well, that is, the Moose  is a puppeteer, and Nny is his puppet, which he controls, forcing him to get blood for the wall. He pulls invisible the strings and Johnny obeys him.
