The titular protagonist of the rhythm game Mad Rat Dead, Mad Rat is, in his words, "A rat that's gone completely mad!" He's a lab rat on the verge of death, granted a chance to relive his final day in order to fulfill his life's dream and die happily... by taking revenge on his human scientist owner for giving him such a dismal existence. Teaming up with Heart, his literal (and sapient) heart with a musical beat, he embarks on a rhythm based adventure that takes him far beyond the laboratory in which he spent his life.

Thanks to his mysterious Heart, Mad Rat possesses the ability to rewind time, which he puts to full use in his mission to get revenge. Vengeful as he is, this rat's also got a softer side to him, choosing to show compassion at multiple points throughout the game. Including, if the player chooses, to the scientist himself...
