how bad does someone have to be to become a demon? are most people turned into demons once they die, and it is rare for mortals to turn into demons while they are still alive? like if someone commits murder they don't turn into a demon right then and there, it's a when-they-die punishment? a greedy person doesn't spontaneously combust into a demon because they stole one too many pennies?

No, the punishment is for when a soul passes over, though the continued existence of the Great Fiends preverses this intention. The design of the Speaker was a place for souls of the wicked to go to repent and face their sins of life. The forms were still quite grotesques, reflections of sin in life, but not to the extent they appear now. The current forms of demons are in line with the macinations of the Great Fiends, who harbor and incite these sins in the dead and the living, preversing them further in death and leading them to the cusp in life. It is only through worship and fellowship with these creatures do we see mortals becoming demons then before their death.
As for the line on might cross to end in that realm, this has been something of debate for...centuries, I would think. There is no one opinion or one understanding of what absolute morals are, even among the Hantheer. There is always something never thought of, the world is after all always changing. What may have worked to maintain order a century ago may no longer work as we progress in our understanding of our worlds.
