This is an extra difficult case for a few reasons: First, laying a flap over the buccal exostosis can create tears in the mucosa portion as the tissue becomes extremely thin in the undercut portions. In addition, thinning the bone in the apical portion can expose the apex of the root if not careful. Thirdly, we need to remove a large amount of tissue in the gingivectomy portion of the procedure that it can even reach into the frenum and therefore a frenectomy needs to be planned during or even after this procedure. Lastly, not only does the bone need to be thinned but also the soft tissue portion.
