Flight AF447, an Air France Airbus A330 flying from Rio de Janeiro to Paris, disappeared over the Atlantic Ocean with the loss of all 228 lives.
How could a state-of-the-art airliner with elaborate electronic safety and navigation features and a faultless safety record simply fall out of the sky? Airline pilots were once the heroes of the skies. Today, in the quest for safety, airplanes are meant to largely fly themselves. Which is why the 2009 crash of Air France Flight 447, which killed 228 people, remains so perplexing and significant. Najib admits mistakes in MH370 but says lessons to be learned by many
KUALA LUMPUR, May 14:
Prime Minister Datuk Seri Najib Razak, in an opinion piece published in the Wall Street Journal yesterday, admitted that the government made mistakes in the first few days after Malaysia Airlines Flight MH370 went missing.
Although, he said these mistakes did not damn the plane, Najib said there were many lessons to be learnt in light of the tragedy which netted his country and administration much criticism and disdain.
âIn the passage of time, I believe Malaysia will be credited for doing its best under near-impossible circumstances.
âIt is no small feat for a country the size of ours to overcome diplomatic and military sensitivities and bring 26 different countries together to conduct one of the worldâs largest peacetime search operations.
âBut we didnât get everything right. In the first few days after the plane disappeared, we were so focused on trying to find the aircraft that we did not prioritise our communications,â said Najib.
While admitting that it took air traffic controllers four hours to launch the search-and-rescue operation, Najib pointed out that the planeâs disappearance between two countriesâ air traffic controls caused maximum confusion.
âDespite this, the search began about a third quicker than during the Air France Flight 447 tragedy in 2009. Nevertheless, the response time should and will be investigated.
âNone of this could have altered MH370â˛s fate. And I pledge that Malaysia will keep searching for the plane for as long as it takes. We will also continue facilitating the independent investigation so we can learn from any mistakes.
âWe have already tightened airport security, and investigators are looking for other measures to improve safety,â he said.
But according to Najib, there are lessons here not just for Malaysia, but for the global aviation industry.
âOne of the most astonishing things about this tragedy is the revelation that an airliner the size of a Boeing 777 can vanish, almost without a trace.
âIn an age of smartphones and mobile Internet, real-time tracking of commercial airplanes is long overdue.
âAfter Air France 447 crashed into the Atlantic, investigators recommended that the airline industry introduce improvements that would help search teams quickly locate a crash site and reach any survivors. But no action was taken,â said Najib.
He said Malaysiaâs preliminary report into MH370 included a recommendation for the real-time tracking of commercial aircraft.
Najib also encouraged members of the International Civil Aviation Organisation to push for real-time tracking of airliners and said changing communications systems, specifically transponders and Aircraft Communications Addressing and Reporting Systems (Acars), so they canât be disabled mid-air should be considered.
âPolicy makers need to reconsider the capabilities of airlinersâ black boxes. At the moment, the location pingers, which are activated if a plane crashes, last only for 30 days. This should be increased to at least 90 days, as the European Union has proposed.
âIf MH370â˛s black box pinger had lasted for 90 days instead of 30, search teams may have been able to locate the plane by now.
âTodayâs black boxes can only record the last two hours of cockpit conversation. This seems wholly inadequate.
âWhen MH370â˛s black box is finally recovered, the most important portion of the cockpit conversation â the minutes and hours after the plane first vanished â wonât be available.
âGiven that a standard iPhone can record 24 hours of audio, surely the black box should have sufficient memory to record cockpit conversation for the full duration of any flight.
âAirlinersâ emergency locator transmitters â which emit a distress signal when the plane is in trouble â could also be improved.
âCurrently they donât work very well underwater, and their mandated battery life is just 24 hours,â suggested Najib.
Again reiterating that these changes may not have prevented the MH370 or Air France 447 tragedies, Najib said they would make it harder for an aircraft to simply disappear and easier to find any aircraft that did, which would help reassure the travelling public and reduce the chances of such a drawn-out disaster reoccurring.
âThe global aviation industry must not only learn the lessons of MH370 but also implement them.
âThe world learnt from Air France but didnât act. The same mistake must not be made again,â said the Malaysian Prime Minister.
Mutt's Insights onto Malaysia Airlines Flight MH 370
After seeing all sorts of stories and theories on Tumblr and the news, I'd like to give my... two or more cents on the topic. I am not an expert, but I do research aircraft accidents in my freetime, so I am... informed? Sure why not that. Will be under read more because of extensive details.
So What Happened?
On March 7 (10:30 Zulu time), Malaysia Airlines Flight 370, a Boeing 777-200ER (extended range) with 227 passengers and 12 crew, took off from Kuala Lumpur Airport in Malaysia, bound for Beijing, China. Â About 2.5 hours into the flight, just as the plane reached its cruising altitude, it disappeared from radar. All forms of communication were also lost, and no distress signal of any kind was received from the aircraft.
Where Did it Go?
Contact was lost with Flight 370 over the Gulf of Thailand, in between Thailand, Vietnam, and Malaysia. Â Vietnamese Naval sources believe they saw the aircraft go down on radar, while another radar contact was assumed to be the plane hundreds of miles off course, back over Malaysia.
The aircraft only had enough fuel for 7.5 hours of flight, so no matter what, the plane is long downed. Many have come to the conclusion that it crashed at sea- it would have most likely been seen on radar, or sent a mayday, if it went down over land.
Why Haven't We Found Anything?
Good question. I don't know.
Ships and planes from 12 countries, including two United States Navy destroyers, are searching for the lost airplane. At first, the search was over the Gulf of Thailand where the last contact was, but then switched to the west side of Malaysia, in the deeper but narrower Strait of Malacca. Nothing directly tied with Flight 370 was found, although oil slicks were discovered on the Gulf of Thailand. And, today, China released information that one of their satellites found large objects floating not too far from the aircraft's flight route. We don't know if they're part of the plane yet or not.
So What Happened?
That's anyone's guess, but here are the preliminary theories:
Terrorism: eyebrows were raised when it was discovered that two stolen passports were used to board the flight. An in flight bomb would also explain no communication from the crew- see Pan Am Flight 103 as an example. However, the two passports were tracked to the individuals who had stolen them, and neither had any background to any terrorist organizations.Â
Pilot Error:Â This is another possibility. At night, it could be very easy to become disoriented and lose control of the plane. However, the captain had 31 years of experience, and had a flight simulator in his house, so I don't buy this one.
Mechanical Failure: The 777 was 11 years old when it was assigned to Flight 370. Usually, planes are in the air for around 22 years or so before retirement.Â
A design flaw is unlikely- the 777-200 has only had two previous accidents; British Airways Flight 38 in 2008 crashed short of Heathrow Airport, but this was due to fuel starvation in the engines designed by Rolls-Royce, and Asiana Flight 214, which crashed last summer in San Francisco with 3 deaths. I don't know what caused this one, but it seems to be pilot error.
Maintenance issues, however, could be at stake. Boeing issued statements about corrosion in the airframes of 777s, and Malaysia Airlines did receive the notification. They're looking right now at the plane's maintenance records to see if it was looked at. An in flight breakup would also explain no signals- see China Airlines Flight 611.

What about the phantom calls?
I saw an article on that today- what basically happened was that when you dial a cell phone number, the network gives rings to the sender while it searches for the other phone. So, it is possible that those calling passengers on the plane could hear ringing, but the phones they are trying to call would not be active.Â
How does this related to Air France 447?
They were advanced aircraft that disappeared.
There are some large differences, though:Â
- The Air France flight, an Airbus A330, was flying through rough weather when its pitot tubes- devices used to give airspeed data to the pilots- froze over for a minute. MH 370 was flying in clear weather.
- The Air France aircraft itself sent automated text messages to Airbus headquarters informing them about failures and warnings and, chillingly, a high vertical rate of descent. Nothing came from Flight 370
- The area of ocean AF 447 vanished in was mountainous underwater terrain 3 miles deep. The Gulf of Thailand is 230 feet deep at its deepest, with an average of around 150 ft, making recovery far easier.
- Debris was not found of AF 447 until the 5th day. We are seeing possible signs of Flight 370, but it's still the 4th day of searching.

The âFinal Reportâ of the French BEA, who undertook the investigation of the Air France 447 crash into the Atlantic on 1 June 2009, comes to some very important and frankly frightening conclusions. It is clear that what happened was a deadly combination of crew under training and several serious mechanical and design failures. Some of these can be traced directly to U.S. companies who supplied components for the Airbus.
 In retrospect, the sequence of events is now pretty clear. And even more clearly is that none of this needed to have happened.  Trouble started when the flight crew elected to continue on its initial course directly across and over the Atlantic, Brazil to Dakar, which would take it into an area of weather where icing could be expected. The sensors on the aircraft then either did not pick up the fact of the icing, which, if severe enough, can disrupt the lift capability of the wings, but here apparently caused erroneous readings to be fed into the autopilot system by partial blocked pitot tubes,
 As a result of this, the autopilot got conflicting information, one source saying it was going to fast and needed to slow down, the other just the reverse. In a situation such as this, the autopilot is supposed to disconnect itself, so that the fight crew can immediately take over and âhand toolâ (fly manually) and act quickly to diagnose and deal with the situation. They did not. The captain, the senior pilot, was not at the controls, and the two junior officers did not respond correctly. One read the aircraft as going too fast, which if continued, could itself be catastrophic. The window of speed though which a swept wind modern jet aircraft must fly at high altitudes is really very narrow; too fast and there can be structural damage; too slow and the plane can stall. (âStallâ here is in the aerodynamic sense, not referring to the engine power; that is. the wings cease to provide lift.)
 In the AF447 case, the pilot at the controls lifted the nose up, thinking he had to slow down the speed. As it turns out, this is the worst possible thing that could have been done. At that altitude, the swept wing 500mph plus jet will stall very quickly if what is called the angle of attack, that is the flow of air over and under the wing, is raised even bit too much. The only way out is to drastically lower the nose and try to increase flying speed, but once the stall is incurred, it may not be possible to recover there from, especially in a high performance jet. That is what happened here; the machine just keep sinking toward the water, but apparently never did roll over.
 A true tragedy. There was litigation initiated in the United States against the US component manufacturers as well as AirBus.  In all but two cases, involving Americans, Air France could not be sued in the U.S. because of jurisdictional restrictions in the governing Aviation Convention. However, a federal court in San Francisco dismissed all of the cases in favor of a transfer to the French courts.
 It is reported now that many of the passenger cases have now been settled by the insurers for Air France, who are now looking to Airbus and the component makers for contribution to the costs of the disaster.
I honestly had panic attacks reading the chain of events which led to the crash of Air France 447: failure to research weather conditions/suitable flight patterns beforehand, pilot inexperience, and failure to communicate between pilots. "For more than two years, the disappearance of Air France Flight 447 over the mid-Atlantic in the early hours of June 1, 2009, remained one of aviation's great mysteries. How could a technologically state-of-the art airliner simply vanish?
With the wreckage and flight-data recorders lost beneath 2 miles of ocean, experts were forced to speculate using the only data available: a cryptic set of communications beamed automatically from the aircraft to the airline's maintenance center in France. As PM found in our cover story about the crash, published two years ago this month, the data implied that the plane had fallen afoul of a technical problemâthe icing up of air-speed sensorsâwhich in conjunction with severe weather led to a complex "error chain" that ended in a crash and the loss of 228 lives.
The matter might have rested there, were it not for the remarkable recovery of AF447's black boxes this past April. Upon the analysis of their contents, the French accident investigation authority, the BEA, released a report in July that to a large extent verified the initial suppositions. An even fuller picture emerged with the publication of a book in French entitled Erreurs de Pilotage (volume 5), by pilot and aviation writer Jean-Pierre Otelli, which includes the full transcript of the pilots' conversation.
We now understand that, indeed, AF447 passed into clouds associated with a large system of thunderstorms, its speed sensors became iced over, and the autopilot disengaged. In the ensuing confusion, the pilots lost control of the airplane because they reacted incorrectly to the loss of instrumentation and then seemed unable to comprehend the nature of the problems they had caused. Neither weather nor malfunction doomed AF447, nor a complex chain of error, but a simple but persistent mistake on the part of one of the pilots."
