Artificial intelligence is a topic that is now being discussed widely. AI, or artificial intelligence, is the capability of computing systems to be programmed to accomplish tasks AND to learn from those tasks to improve their abilities the next time the system is faced with a problem, question, or decision.

We’ve all heard about self-driving cars, voice recognition technology, face recognition technology, and of course the application of AI to the field of pathology in medicine. Many people in our industry have taken on the challenge of programming computers to create algorithms that can review human pathology slides, find rare events (isolated micro metastases), spots of dedifferentiation or stromal changes inside a tumour that may predict biologic behaviour, or help automate repetitive and/or laborious tasks in slide reviews.

Of course, the worry that pathologists will be replaced by machines has come up at every conference and bar discussion on the subject. We in the community of digital pathology, however, have a different perspective. We see AI as a tool that will help us perform what we already do more effectively, more quickly, and more affordably. Over time, accuracy may also increase.

How pathologists will use all the extra time to generate the same or more money with support from automation is, in my opinion, one of the many questions regarding AI in pathology that will need to be answered. Consider the possibility that AI might increase our productivity by 20–30%. Does that imply a weekly day off from work? additional time to complete papers? or instruct? Or perhaps you should return to the Wednesdays you used to play golf and meet with your accountant?

Or pathology will become even more commoditized through automation and price constraints. Will volume-based remuneration continue to be paid rather than being boosted to account for the additional CPTs and RVUs. When calculating reimbursement, will Medicare and commercial insurers decide that a computer is less expensive than a doctor?

Or will automation allow those of us taking the place of the ageing pathologists to make up for the rise in our workload, the ageing pathologist population, the decline in the number of medical students entering the field, and the lack of new residencies in the near future? We can generate twice as much with AI, which will help us meet the increased demand for our services.

This creates the ideal time for AI in pathology since it is not a question of whether we want to utilise it or not; rather, we must embrace it.
