October is a busy month for DIDD’s Planning and Policy Councils.  Here are the upcoming meetings.

Last week, we shared that Policy 80.4.7, Community Transition, has been updated for organization, clarity, and compliance with the Centers for Medicare and Medicaid Services (CMS) Home and Community Based Services (HCBS) Final Settings Rule.  There will be an opportunity for training on the Community Transitions Policy revisions.  The date and time set aside for initial training is August 17th from 1 p.m.-3 p.m. CDT.  This will be conducted via Web-Ex and participants will have the opportunity to ask questions using the chat feature.  You may register by clicking here.  After your request has been approved, you'll receive instructions for joining the meeting.  You may join the meeting up to10 minutes prior to the start time.  Please contact [email protected] with questions or concerns.  Also, you may view the policy and accompanying documents on the DIDD website.

Policy 80.4.7 Community Transition has been revised for organization, clarity, and compliance with the Centers for Medicare and Medicaid Services (CMS) Home and Community Based Services (HCBS) Final Settings Rule.  Language pertaining to transition planning meetings; contested transitions; inspection/licensing requirements for Supported Living (SL), Semi-independent Supported Living (SILS), Residential Habilitation, and Family Model Residential Services; requirements to work with the Circle of Support (COS) to decide if a Therapeutic Assessment is necessary; provider requirements for paperwork to be submitted to the Regional Office/Central Office following transitions; and Independent Support Coordinator (ISC) visitation requirements has been clarified.  “Natural Supports” and “Non-Routine Events” are defined in this revision.  Language has been added to correspond with CQL Network Accreditation Performance Outcome Measures (POM) plan for personal outcome: People Choose Where and with Whom to Live.  Language has been added to correspond with CQL Network Accreditation BA plan for Network factor 3A: Policies and practices facilitate continuity of natural support systems.  Language has been added regarding COS mindfulness in setting timeframes for transitions to have transition plans submitted to the Regional Transition Unit fourteen (14) days prior to the expected move date.  Language has been added regarding environmental modifications, as well as Exception to the Community Transition Process.  Language has been added regarding transitions being in accordance with the Provider Manual Chapter on Residential Services prior to transition to a new residence.  Language has been added regarding when requirements for a person to remain in a rented/leased residence or room and board residential setting where a tenancy agreement is in effect may be waived.  The definition for “exploitation” has been removed from this policy, as well as language regarding recruitment.
