Arganan knows that he's very visibly disabled and that his physical disability obviously affects his everyday life. He has to rely on a cane majority of the time, and he's a canonical multi-limb amputee.
I've discussed his disability and my headcanons around them before in these two posts (post 1 and post 2). However, to add to this,I will say that it's implied that he may have tried to be a soldier before (before his brother died and Arganan became the Count of Lazulis Island, Arganan didn't have any important position otherwise) but lost that chance due to his physical disease and disability. So there's definitely some internal shame he's struggled with (though he's working on accepting himself as is).
He does his best to look as nice and neat as he can appearance-wise- it's kind-of needed in a position like his. However, my post-game Arganan (basically he survived whatever should've canonically killed him but was physically weakened, and Calista took over as ruler of Lazulis) is a little less worried about appearances and more just focused on making sure he maintains a decent-enough state of health.
Also, reputation-wise, I seem to have developed the running theme of everyone else thinking that Arganan "went mad" during the end of the war between Lazulis and the Gurak, hence why Calista took over being ruler of Lazulis immediately after the war ended - however in reality that is false. Arganan is annoyed by that but also uses it to his advantage to not always look 100% polished.
