I knew Lucy Knight wasn't leaving ER alive and nothing could have prepared me for The Horror of her last episode, oh my god.
I also knew Carter had something to do with it in some capacity because I saw people pointing at him being guilty to some capacity and it's true she spends her second-to-last episode trying to get his attention over the patient, but it feels a little unfair that only him seems to be pointed at when there are several mistakes commited by different doctors that could have prevented her demise and almost everyone failed her (and Carter too actually, because someone should have found them earlier).
I would be angrier if I were Kerry (she is so loved by me, you have no idea). She's even mad when Mark and Luka are telling her about the patient because it was truly a mess in general, she doesn't blame Carter in particular.
It's quite similar to how I felt in episode 1x19 with Mark. Yes, Mark there and Carter here fuck it up, but so did others. It's just they have the most prominent and direct role in the respective tragedies.
