Tomura is the first one to notice and when he does all he can ask is "Why?" Or "What do you want?" Because why would he ever expect anyone to ever do things for him or others without wanting something in return. To him nothing ever comes without a price. He doesn't get it because truthfully Spinner wouldn't want anything but for all of them, including Tomura most of all, to just be happy.

tomura feels so far removed from tenko that the name doesnt even bother him that much. that child has been dead for so long that he can’t go back, nor does he want to. even if the name tomura was bestowed to him by the man who made him a killer, it still feels more real than the sobbing child haunting him. shimura though? he could do without that mantle

IIRC there's another cutscene in this game with Twice catching a cold, and Tomura's first thought is: When you're sick you need nutrition > Meat is the best nutrition > We're ordering grilled steaks. (lol) That and remembering that Compress likes sushi-- I like to imagine he was copying how Kurogiri treated him.
