Definitionally he does not realize what he’s doing; the whole point is that he has unconscious misogynist biases informing his worldview that lead him to instinctively assume comparative ignorance on the woman’s part. It’s the “I can’t operate, this is my son” joke writ large—a systemic bias that women’s ignorance and lack of authority can be safely assumed unless proven otherwise.
