As for PTSD, it seems to me that the game pretty clearly hinted that Bill might suffer from it or some sort of anxiety disorders (I guess everyone remembers that interaction by the campfire when Bill talks about the Indians and got scared by the horses neighing, right?). Whether it’s war-related PTSD or just generally from his time in the army, I’m not sure, because we don't know much about the times of his service. the game doesn't go into details, sadly. He himself claims that he 'saw things,' so maybe he really does bear signs of some kind of trauma.

As for headcanons; honestly, I haven't really thought about it that much and I mostly base them on what we can see during gameplay, even though it's not an optimistic scenario. Bill does not have healthy coping mechanisms, that's just a fact.
